Abstract
Fibroblast activation protein inhibitor (FAPI)-based radiopharmaceuticals have emerged as one of the most promising innovations in contemporary nuclear medicine and precision oncology. Fibroblast activation protein (FAP), overexpressed in cancer-associated fibroblasts (CAFs) within the tumor microenvironment of many epithelial malignancies, has become an important molecular target for both diagnostic imaging and targeted radionuclide therapy. In recent years, FAPI radiopharmaceuticals have demonstrated outstanding diagnostic performance characterized by rapid tumor uptake, low physiological accumulation in healthy tissues, and excellent tumor-to-background contrast in positron emission tomography/computed tomography (PET/CT) imaging. Furthermore, therapeutic radionuclides such as Lutetium-177, and Actinium-225 have enabled the development of FAPI-based radionuclide therapy, expanding the role of FAPI compounds from diagnostic imaging to therapeutic oncology. This review discusses the biological basis of FAP targeting, the development and radiochemistry of FAPI radiopharmaceuticals and their current clinical applications in diagnostic imaging and targeted radionuclide therapy. Among other aspects, we aimed to highlight the major limitations, current challenges, and future perspectives of FAPI theranostics in precision medicine.
