Abstract

Motor neuron disease comprises progressive neurodegenerative disorders affecting upper and lower motor neurons. Mills’ syndrome, the hemiplegic variant of amyotrophic lateral sclerosis, is a rare phenotype characterized by unilateral spastic hemiparesis without sensory deficits. We report a 50-year-old woman with a two-year history of progressive left-sided motor impairment, initially presenting with lower back pain followed by stiffness and weakness, leading to severe gait impairment. Neurological examination revealed spastic hemiparesis, hyperreflexia, and preserved sensory function. Magnetic resonance imaging and cerebrospinal fluid analysis were normal. Electromyography demonstrated widespread chronic neurogenic changes, while other investigations excluded alternative diagnoses. These findings support the diagnosis of the hemiplegic variant of ALS (Mills’ syndrome).
