Abstract
Diabetes mellitus represents one of the most prevalent chronic diseases worldwide and is associated with a considerable health and economic burden, primarily due to its metabolic and cardiovascular complications. Insulin resistance (IR) is a key mechanism in the pathogenesis of these disorders, while the triglyceride–glucose (TyG) index has been proposed as a simple, practical, and reliable indicator for its indirect assessment. The aim of this study was to evaluate the role and clinical validity of the TyG index in patients with diabetes in the Tetovo region, as well as to analyze its association with metabolic parameters. This study is a retrospective investigation and included a total of 251 participants, of whom 201 were patients with diabetes mellitus, and 50 were healthy individuals serving as the control group. Blood samples were collected in the fasting state and analyzed for glucose and triglycerides using standard laboratory methods. The TyG index was calculated according to the formula ln [TG (mg/dL) × FBG (mg/dL) / 2]. Statistical analysis was performed using IBM SPSS software, with the level of statistical significance set at p < 0.05. The results showed a statistically significant increase in the TyG index in the diabetic group compared to the control group (8.91 ± 0.62 vs. 8.64 ± 0.29; p = 0.003). Furthermore, a progressive increase in the TyG index was observed in relation to fasting glucose levels, particularly in patients with hyperglycemia ≥ 6.1 mmol/L. Correlation analysis revealed a positive and statistically significant association between glucose and triglycerides in diabetic patients (r = 0.234; p = 0.001). In conclusion, the TyG index represents a valuable, cost-effective, and easily applicable biomarker for the assessment of insulin resistance and metabolic disturbances, supporting its use in the early identification of high-risk individuals and in optimizing the clinical management of diabetes mellitus.
